We're delighted you've chosen to book with us. Please review our policies below as booking an appointment indicates our agreement to these terms.  ​

​ Intake Form

• Complete the New Patient Intake Form at least 24 hours before your appointment

• Access the form through your appointment confirmation email

• Keep us informed of any changes in your health status

 Cancellation Policy

• 48-hour notice required for cancellations or rescheduling

• Late cancellations (less than 48 hours) or missed appointments incur full service charge

•  1.  Notify us immediately 
  2.  Cancellation fee applies if we cannot fill the vacancy (someone to take your spot)
  3.  Exceptions my be granted at Yaimayu Massage Movement , PLLC 's (YMM) discretion​For illness or emergencies within 48 hours:
   

 Appointments Attendance

• Late arrivals may result in shortened session at full cost

• ​Session length may be adjusted to accommodate subsequent appointment

• Full payment required regardless of session duration

• Policy applies to all circumstances including illness, weather, or emergencies

 

 Payment Security​

• Valid credit card required to secure booking

• Card charged only for cancellation/no-show violations

• Alternative payment methods accepted at time of service

 Communication

• We welcome feedback about your preferences and needs

• Please inform us immediately of any discomfort during sessions.

• Both positive and constructive feedback are appreciated

 

 Insurance Patients (Health insurance, Car accident, Workers Comp)

• Requirements:
  1. Valid prescription form qualified provider required
  2. Prescription must be updated annually if no expiration date shown
  3. Insurance agreement is between patient and insurance company                 

• Payment Terms:
  1. Patient responsible for denied claims
  2. Payment authorized to YMM
  3. Balance due from patient if insurance doesn't pay within 60 days
  4. Collection fees apply for unpaid balances
  5. 48-hour cancellation policy applies (not covered by insurance)
